With significant healthcare reform initiatives such as the adoption of Patient-Centered Medical Home certifications for primary care practices, there is a growing need to electronically connect and communicate between disparate PCP and Specialty practices to make sure patient management is done with quality, cost and coordinated care in mind. In addition, all large integrated health systems are determining the best ways to keep their Primary Care physicians referring to the system’s specialists for coordinated care and to prevent “referral leakage”. So the need for streamlined referral management is for all types of physician practices. This includes independent practices and those that are employed directly by the health systems. As a start-up in Western Pa, we are actively building referral networks for independent doctors as well as UPMC and Allegheny Health Network employed and affiliated practices.
