As a nephrologist, I often receive consult requests for incidental low bicarbs, say around 18, but the rest of the chemistry is normal. And the patient comes in for something seemingly unrelated, like chest pain rule out.
What should I do in these cases? Is it worthwhile to ask for an ABG, though patients really don't like getting ABGs, which are particularly painful.
