Falls interventions today is based on population studies and does not meet the individual needs of the patient. The early uptake of apple healthkit in hospitals , the success of mhealth platforms like those of Kaiser Permanente and advent of mhealth and wearable sensors -- make it increasingly possible to collect and integrate CONSUMER CENTRED data and tailor interventions to suit the needs of the consumer. I propose using sensors that measure BP /HR and gait to harness information about the patient on a mobile based app. This information will be stored on patient centred cloud , will be platform agnostic and integrate data about his medications and medical history from acute and community healthcare settings.Such an app will help prevent falls and prevent hospitalizations from early signs of gait instability / abnormal vitals gathered from the sensors, and enable patient specific interventions , and also promote carer /provider engagement .
