Care at Hand is an evidence-based technology that has been shown to reduce hospital readmissions using a mobile care coordination application. Non-medical workers, such as health coaches, home care workers, or office secretaries, use the app to administer “smart surveys,” which detect precursors of health decline. The application presents questions using straightforward, non-medical language based on patient's diagnoses, treatment, and risk profile. The app notifies the patient’s clinical care team of the increased risk in real time, providing an opportunity for timely intervention before the patients condition deteriorates.

After implementing Care at Hand, Elder Services of the Merrimack Valley, Inc. saw a 39.6% reduction in their consumers’ 30-day readmissions.

Care at Hand creates real opportunities to support older adults and individuals with disabilities to age in place.
