The primary clinic would incorporate primary care providers, clinical pharmacists, nurses, dentists, medical social worker and students from each respective discipline serving underserved patients. The coordination of care will be facilitated by a nurse patient advocate (NPA)who coordinates all services. This model provides patients with a more personal, holistic experience the day of their visit. Patients do not have to make separate appointments with each discipline but will be referred to the appropriate discipline the day of their primary care visit. The NPA is the one person who both patients and the other health care providers in the clinic can consult when facilitation of care and follow-up are needed. When a representative from a requested discipline is not physically in the clinic, telehealth services will be initiated for the patient to speak directly with the health care provider utilizing electronic technology.
