Effective team care hinges on robust collaborative working relationships which begin with awareness and recognition of other professionals’ skills and abilities, and expand to a commitment to collaborative working relationship only after exploration and trial with each other. Family Medicine physician residents, Family Medicine pharmacy residents, social work, nursing, medical and pharmacy students can learn about each other while providing comprehensive care through home visits. Home visits reach patients who may otherwise not recieve care in a setting where they are most comfortable and allow for more thorough evaluation of patients social situation as well as an opportunity to develop relationships with family members and caregivers. A home visit curriculum through Family Medicine Residency Programs, with either on-site or visiting learners from other professions provides a forum for exploration of each other's knowledge base and skillset as they see patients in their homes.
