Core temperature is among the vital signs routinely measured in hospitalized patients. Accurate core temperature measurements are particularly important for patients having general anesthesia because anesthetic drugs markedly impair normal thermoregulatory defenses, and the consequent hypothermia causes numerous severe complications.
According to the CDC latest guideline for the prevention of SSI published in this May, maintain perioperative normothermia is a category IA recommendation, meaning that normothermia is strongly recommended during surgery. However, there are no recommendations on how to achieve and maintain normothermia, what’s the lower limit of normothermia, and what’s the optimal timing and duration of normothermia for the prevention of SSI. The issues are still remained unresolved. The problems here are redefine normothermia, and evaluate different strategies of normothermia.
On the other hand, over half of the SSI happens after discharge about 10 days later. The patients are supposed to be monitored closely of SSI signs and symptoms. However, there is a care gap between providers and patients. We interviewed some day surgery centers and basically they stop following up with the patients after 3 phone call attempts during the first 3-7 days after discharge. The problem here is how to facilitate the remote monitoring to provide seamless communication between providers and outpatients.
